Dear Parents/Guardians:
The Individuals with Disabilities Education Improvement Act (2004) requires that a Needs Assessment Survey be conducted annually for the purpose of planning training opportunities for staff and parents. Your participation in this survey is voluntary.
The survey results will be compiled and shared with our eleven member districts for the purpose of planning workshop opportunities. Your time and input is greatly appreciated.
An electronic survey is available (in English, Arabic, Spanish, and Polish). Paper-based version Urdu is available.
If you are completing a paper-copy of the survey, please return it your child’s teacher. Your child’s teacher will deliver the paper copy of the survey to Maggie Sheldon at A.E.R.O.
